What the I-693 immigration medical exam includes

The USCIS immigration physical is a specific exam required by U.S. Citizenship and Immigration Services. It is not the same as an annual physical. Only a USCIS-designated civil surgeon can complete Form I-693. At your visit, Dr. Gutiérrez performs and documents:

Do not open the sealed I-693 envelope before submitting it to USCIS. An opened envelope is invalid and USCIS will reject it. Bring the sealed envelope to your interview or mail it with your response to a Request for Evidence.

Bilingual immigration exam — Spanish

Dr. Gutiérrez is fluent in Spanish. Your entire I-693 exam — history, physical, counseling on vaccines, and instructions for submitting the sealed envelope to USCIS — can be conducted in Spanish. Patient forms and consent documents are available in Spanish as well. See our página en español.

Do I need an I-693 for every immigration case?

Form I-693 is required for most adjustment-of-status applications (Form I-485) filed inside the United States. It is not required for consular processing outside the U.S., which uses a separate exam conducted by a panel physician abroad. Your immigration attorney can confirm which applies to your case.

How long is the I-693 valid?

Under current USCIS policy, a properly completed and signed I-693 is valid indefinitely as long as it was properly signed no more than 60 days before filing the Form I-485. If your I-485 has already been filed, the I-693 remains valid through adjudication. USCIS policy can change — verify with your attorney before scheduling.

What if I don't have my old vaccine records?

If records cannot be obtained, the civil surgeon can order blood titers to check immunity for certain vaccine-preventable diseases (measles, mumps, rubella, varicella, hepatitis B) or provide the missing vaccines at your visit. We will explain your options at the appointment.
