What to bring to your DOT physical
- A photo ID or driver’s license
- A complete list of your current medications, including doses and prescribing providers
- Glasses, contact lenses, or hearing aids if you use them
- Any specialist letters relevant to your health conditions
- Your CDL number, if you already hold one
If you have a condition such as diabetes, high blood pressure, sleep apnea, or a heart condition, bring recent records or treatment documentation from the treating provider. Having this at your visit helps avoid a follow-up appointment.
